Mandate
To establish guidelines, criteria and rules to govern the collection, use and disclosure of personal information obtained by OakTree Golf club from its members, customers, guests, employees and other individuals.

Policy
In the course of its operations, OakTree Golf Club must collect information from its members, guests, employees and other individuals to facilitate the provision of its services and products. Such information may include the names, addresses, birth dates, credit card information and particulars, handicaps, playing experience, product preferences and other information, and in certain instances, such as employee personnel issues ,limited personal health information of individuals. Except for uses properly required by law, OakTree Golf Club will not knowingly convey such collected information to other parties without first obtaining the consent to do so from those providing the information and will take reasonable precautions from inadvertently disclosing such information and records without such consent.

Definitions: For the purposes of this policy:

“Personal Information” means information about an identifiable individual, but does not include the name, title or business address or telephone number of an employee of an organization.

“Record” includes any correspondence, memorandum, book, plan, map, drawing, diagram, dictorial or graphic work, photograph, film, micro-form, sound recording, video tape, machine-readable record, and any other documentary material, regardless of physical form or characteristics, and any copy of any of those things.

Unless otherwise specified, the terms “Personal Information”, and “Records” will be collectively referred to as “Information”.
